Dre takes the family to Walt Disney World and wants the kids to have the experience he never had, so he gets them a VIP tour. The kids get spoiled from bypassing lines all day and, when their VIP time is up, they get bratty when they now have to wait like everyone else. After calling them out on their ungrateful attitudes, Dre has an outburst and leaves them. Meanwhile, Bow ends up spending the day with Dre's parents since she can't ride most of the attractions due to her pregnancy.

As Otto Gerhardt is being taken to a doctor appointment, Simone has sex with Mike, inadvertently mentioning the doctor visit. Bulo phones his superiors, who reject the Gerhardts' proposal and now offer two million dollars less than previously and demands the Gerhardts' complete surrender. In Luverne, Hanzee's investigation leads him to find Rye's belt buckle in the Blumquist fireplace. Lou talks to the Blumquists regarding his suspicions that they are involved in Rye's death, but they stubbornly refuse to cooperate. He warns them of the Gerhardts' violent history. At the Gerhardt farm, Floyd tells the family to prepare for war.

Conor requests that the Donovan family come together for his birthday, which Ray and Abby have forgotten; Ashley's stalker returns; Mickey's gift angers Daryl and strains his relationship with Claudette; An awkward incident leaves Bunchy confused; Terry learns an awful secret about his gym.

April steps up as Chief Resident while managing a clinical trial. Callie and Cristina deal with a difficult spinal surgery while Alex and Teddy must find alternative ways to operate on a patient. Meredith struggles not to interfere with Derek at the hospital.

When House discovers that the experimental drug he has been using causes fatal tumors, he decides to attempt to excise them himself. However, he can not complete the surgery and ends up needing Cuddy's help. Meanwhile, Thirteen's friend from prison, a relapsed drug user, arrives at her apartment needing medical care after being stabbed. With her friend unwilling to go to the hospital, Thirteen enlists Chase's assistance when the friend loses sensation and movement in her arm. Also, Taub receives some unexpected news that could change his life.

Max discusses about with Tara about new alter, causing her to feel uncomfortable and nauseous at the same time (due to Buck's smoking, which Tara isn't familiar with). This not only transitions her into Alice, but also making her happy. Max decides to embrace his time with her, even though he misses his wife. Alice believes she is pregnant shortly after she re-emerges (thinking Tara's vomiting due to Buck as morning sickness), and is later distraught when she thinks she has miscarried, but Max reassures her it is just Tara's period. Despite Marshall's concerns that Jason's father knows about his crush on Jason, the boys grow closer.

Bailey is promoted Attending Surgeon by the chief. Dr. Dixon suggests that Dr. Bailey will make a good pediatric surgeon. As Derek finally decides to propose to Meredith, Addison calls him, and he decides to postpone the proposal. Dr. Arizona Robbins hits on Dr. Callie Torres.

Alex is offended when Izzie talks to Meredith about how great of a man Alex is. Derek is stabbed with the scalpel in the palm of his hand by Rose, while the Chief is stunned over Mercy West's increase in the number of patients, a deadly ice storm ravages Seattle.

As the team rushes to find the underlying cause of Amber's injuries, clues inside House's head may hold the key and House's friendship with Wilson is tested as memories from the bus accident the night before threatens their relationship. Meanwhile, Thirteen struggles to cope with her own personal problems while treating Amber.

Mark decides to turn over a new leaf, and refuses Callie's invitations for sex. Meredith fires her therapist, Dr. Wyatt, who tells her that she is a quitter.
