Mystic Falls begins the preparations to celebrate the passing of a comet. While Vicki recuperates in the hospital from her injuries caused by a vampire, Stefan uses his abilities to erase Vicki's attacker from her memory, but is interrupted by Matt. Mr. Tanner warns Jenna about Jeremy and tells her that she is doing a bad job of taking care of teenagers. Elena and Damon meet for the first time at the Salvatore house and his revelation about Stefan's past brings Elena closer to the truth. Elena and Stefan kiss. Damon finds his next victim in Caroline.

Elena Gilbert and her brother Jeremy can't come to grips with the fact their parents have died. But nothing goes as planned for both of them when mysterious new student Stefan Salvatore attracts Elena's attention, leaving her ex-boyfriend Matt and her rival Caroline unaware of what's happening. After Matt's sister Vicki is bitten by something they believe to be an animal, it's discovered that Stefan is a vampire. Stefan (who does not feed on humans) realizes he is not the only vampire in town. Stefan's evil brother Damon enters the picture; they have not seen each other for 15 years.

After the events in the On-Call room, J.D. and Elliot continue to question their respective commitments to Kim and Keith. Carla tells Turk that due to his diabetes, he can only have one candy bar every six months. Dr. Cox struggles to diagnose a patient. Meanwhile, Janitor may have a new girlfriend.

Having just returned from her honeymoon with Meredith, Cristina searches for Burke, but he is nowhere to be found. Meredith, Cristina, Izzie and Alex spend their first day as residents with their own groups of interns. Among the interns are George, repeating his intern year after failing his exams, as is Lexie Grey, Meredith's half-sister whom she has never met. Now that Derek's relationship with Meredith has reached an emotional impasse, he looks to his fellow doctors for friendship; Bailey wrestles with her place within the hospital now that her former interns report to new Chief Resident Callie; and Richard resumes his position as Chief of Surgery.

The O'Malley Family visits George at the hospital, Meredith is determined to have a more positive outlook on life, Derek and Addison strive for a civil relationship, and Richard endeavors to reunite with his wife, Adele.

Cristina meets Burke's parents, in the most awkward of circumstances; Izzie begins baking to comfort herself; Addison takes a break from her hospital duties and gets drunk; Richard discovers Callie living in the hospital; and a surgical patient lives what may be her last day to the fullest. Meredith makes a proposal to Derek and Finn.

The episode begins with Meredith running out of Seattle Grace, choosing neither Finn nor Derek. The interns help Izzie cope with the loss of her fiance, Denny. Meredith begins to deal with the aftermath of the prom, concerning the Finn/Derek love triangle. Addison, devastated, discovers a pair of black panties in Derek's tuxedo jacket and knows the owner is Meredith. She washes them and then posts them beneath a "Lost and Found" sign in the hospital hallway. Addison has a sexual relationship with Derek's best friend, Mark Sloan. Derek went to end his and Addison's relationship. He went to the hotel room Addison was staying at, telling her that their marriage is over. From the bathroom emerges a naked Mark Sloan with a towel. Derek is shocked and leaves. Derek makes no secret of his desire to end his marriage and be with Meredith. In the end, both men tell her they will wait for a decision. The Chief is confronted by his wife Adele and is given a very difficult decision (retire or divorce) that he must soon make. George and Derek get quarantined when there is an outburst of the plague at Seattle Grace. Derek gives George relationship advice concerning Callie. After speaking with the Chief, Cristina realizes maybe she has taken her relationship with Burke for granted. Flashbacks happen, including when Derek meets Meredith, when Addison is caught cheating on Derek, the mixer the night before the interns begin their first shifts, and when the Chief ends his affair with Ellis Grey. Meredith has to choose between Derek or Finn.
