Monogamy, directed and co-written by Dana Adam Shapiro, is about the strained relationship of an engaged Brooklyn couple, Theo (Chris Messina) and Nat (Rashida Jones). Theo is bored with his job as a wedding photographer-the generic backgrounds, the artificial posing, the stilted newlyweds-so he develops an unconventional side business, called "Gumshoot," a service where clients hire him to stalk them with his camera. Becoming infatuated with one of his clients, a mystery woman who goes by the name Subgirl (Meital Dohan), Theo develops a voyeuristic obsession that forces him to confront uncomfortable truths about himself and his impending marriage.

Derek fails in his attempts to save Heather, leaving Shane feeling responsible for his colleagues death. The interns are tasked with comforting Heather's mother when she arrives, but realize that they had been bad friends to her, and resolve to look out for each other more. Bailey and Catherine clash over how to proceed with Richard's condition, with Bailey wanting to perform risky surgery. Meredith ultimately accepts responsibility for Richard's care and allows Bailey to follow her instincts, saving Richard's life. Jackson rejects April following her declaration of love and she decides to go ahead with her engagement to Matthew. Owen and Cristina struggle to stay apart following their split. Arizona tracks Callie down to Meredith's house but Callie is again unforgiving and informs Arizona that they will now take turns caring for Sofia. Arizona later returns, desperate to see Sofia and whilst Callie initially refuses, Cristina urges her to not bring Sofia into the fight, resulting in Arizona being granted five minutes with her daughter.

After Addison discovers that a mentally unbalanced pregnant patient has lost her baby, Violet must convince her that the baby has died and that surgery is necessary, while Cooper must tell a longtime patient he must remove part of his jaw due to a fast-growing cancer. Meanwhile, Dell copes with the absence of his daughter after his ex moves for a new job, and Sam and Addison find themselves suppressing their feelings for two very unattainable people.
