Meredith dreams of a world in which her mother, Ellis Grey, never had Alzheimer's and instead had loving and supportive parents. This causes Meredith to be happy, which causes Seattle Grace to be completely different.

After all the rookies, except McNally fail their gun re-certification, Peck and Epstein are assigned to help a man with amnesia while Nash and Diaz help Detective Barber reconstruct notes that went missing for an upcoming court case. McNally is assigned with Swarek to do a prisoner transport. Although they find their situation awkward, they talk and reconcile. However the inmate escapes the custody of the two officers, leaving them in a frantic race against daylight to find the prisoner.

One of her old acquaintances runs for Lieutenant Governor in Sacramento, and Kristina leaves Max and Haddie with Adam for the weekend. Kristina is excited by the opportunity to help, but feels out of date among the young generation of campaign workers. Meanwhile, Adam has his hands full, and along with taking care of his own kids, gives Drew girl advice as a school dance approaches. Elsewhere, Sarah tries to get Amber excited about college by taking her to an event on campus and unexpectedly runs into an ex. Julia attempts to teach Sydney a lesson about lying. Crosby takes Jasmine on a real date without Jabbar.

Anna pays a visit to Damon, bringing along her mother Pearl, who wants to take control of the town. All the vampires in the tomb have been released, and most are staying at an old farmhouse on the outskirts of town.

The episode begins with Meredith running out of Seattle Grace, choosing neither Finn nor Derek. The interns help Izzie cope with the loss of her fiance, Denny. Meredith begins to deal with the aftermath of the prom, concerning the Finn/Derek love triangle. Addison, devastated, discovers a pair of black panties in Derek's tuxedo jacket and knows the owner is Meredith. She washes them and then posts them beneath a "Lost and Found" sign in the hospital hallway. Addison has a sexual relationship with Derek's best friend, Mark Sloan. Derek went to end his and Addison's relationship. He went to the hotel room Addison was staying at, telling her that their marriage is over. From the bathroom emerges a naked Mark Sloan with a towel. Derek is shocked and leaves. Derek makes no secret of his desire to end his marriage and be with Meredith. In the end, both men tell her they will wait for a decision. The Chief is confronted by his wife Adele and is given a very difficult decision (retire or divorce) that he must soon make. George and Derek get quarantined when there is an outburst of the plague at Seattle Grace. Derek gives George relationship advice concerning Callie. After speaking with the Chief, Cristina realizes maybe she has taken her relationship with Burke for granted. Flashbacks happen, including when Derek meets Meredith, when Addison is caught cheating on Derek, the mixer the night before the interns begin their first shifts, and when the Chief ends his affair with Ellis Grey. Meredith has to choose between Derek or Finn.

Meredith and Derek have to use more than medicine when a young Asian woman's traditionalist, religious father forbids her to have a life-saving operation unless a shaman is called upon to "retrieve her souls". Meanwhile, George and Alex are forced to perform surgery in a trapped elevator in an attempt to save a patient's life in which George shows his potential while Alex shuns the challenge, and it is decision time for Derek and the two women in his life.

Alex treats a teenage Orthodox Jew who refuses surgery when she learns the treatment goes against her beliefs. Cristina has trouble relating to a pregnant woman who has terminal cancer and refuses to abort the baby, turning to Meredith for advice. Izzie is mesmerized by a patient who has psychic abilities, and is diagnosed with AVM.

Derek spends the night at Meredith's, which they try to keep secret from her other roommates. George suspects an anesthesiologist of drinking before major surgeries but is scolded when he does speak up. Alex treats a man who enjoys being in pain, and Cristina catches the flu.

When a retired nurse at Seattle Grace is admitted, Cristina takes on her case and befriends her. A patient who recognizes Izzie from a modeling photo does not want her to treat him. Meredith and Shepherd treat a construction worker who surprisingly survived his injuries, but have bad news for his wife about his future. Alex humiliates Izzie in front of everyone.

Competition between the interns ensues after an annual unauthorized bike race causes many injuries. Izzie has trouble locating the family of a near brain-dead man, and Cristina is excited when the patient's family agrees to harvest his organs if he dies, leading Bailey to lecture her on bedside manner. A patient who is a friend of Dr. Webber flirts with George.
